Overview

Optimising Data To Integrate Health and Social Care in Dublin 8 was one of the three pilot-project proposals selected for the second internal call for pilots launched in March 2021 within the consortium. It addressed one or more of the themes highlighted in the population needs analysis conducted at the outset of the Smart D8 initiative. 

Optimising Data To Integrate Health and Social Care is an initiative to combine data from different sources into a single and unified dataset. 

Introduction

The Optimising Data To Integrate Health and Social Care in Dublin 8 pilot focused on:  

Enhancing local healthcare data connectedness to larger area data holding and healthcare planning organisations 

Identifying data needs for health planning, implementation, and pathways monitoring 

Objectives

It aimed to optimize data use to maximize Dublin 8 healthcare delivery.
