Overview

Heart of Our City was one of the five winning proposals selected for the first call for pilots launched in March 2021, which addressed three key community needs previously identified with the community survey: mental health; population health management; and Covid-19 impact. 

Heart of Our City is a patient empowerment initiative, aimed to improve awareness around the management of cardiovascular disease in the community. 

Introduction

In response to identifying heart health as an area of concern for Dublin 8 citizens, Heart of Our City connected people in Dublin 8 with a range of resources to support their cardiovascular health, such as information on heart health, and heart and stroke support services, building on existing social prescribing initiatives in the locality.  

Objectives

The pilot aimed to improve awareness and management of heart health to reduce the burden on local services and unplanned hospitalisations
